CANCELLATION CHARGE
If less than 48 hours’ notice is given before cancelling your appointment, or if you fail to attend, you will still be charged the full amount for your appointment.

Glasgow City Physiotherapy reserves the right to apply discretion in exceptional circumstances.
DATA PROTECTION
All information provided on your application form will be processed by the clinic in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018.
Your personal data will be collected, stored and used only for purposes directly related to your care and the administration of our services.
CONSENT TO TREATMENT
All assessments and treatments will be performed by a qualified, registered and insured therapist who practises within their defined scope of practice in accordance with the relevant governing bodies in the United Kingdom.
You must advise your therapist of any factor that may affect your treatment or diagnosis. Following examination, an explanation of the therapist’s clinical opinion and proposed treatment plan will be provided.
It is important that you fully understand the diagnosis, recommendations, procedures and treatment techniques that may be involved, as well as any potential benefits and risks. Your therapist will make every effort to ensure you do; however, if you are unsure about any aspect of your care, please ask for further information.
You understand that you have the right to decline or withdraw consent for treatment at any time.
With certain conditions, a degree of undress may be required. This will always be explained to you by the therapist prior to assessment or treatment.
PAYMENT
Payment is required at the time of your appointment unless you have Private Medical Insurance and are able to provide the required details of your insurer prior to treatment. Receipts will be provided if required.
Any outstanding balances remain the responsibility of the patient.
INSURANCE
Glasgow City Physiotherapy is registered for physiotherapy with Axa, Aviva Health Insurance, WPA, Cigna Health Care and Vitality.
It is your responsibility to obtain authorisation from your insurance provider before your appointment. Where possible, we can invoice certain insurers directly on your behalf. Fees charged to insurers may differ from our standard self-pay fees.
Please note that your insurance policy may not cover the full cost of your treatment. You remain personally liable for any charges not settled by your insurer.
You may choose to pay in full at the time of your appointment and manage the claim process yourself.
AGE
If you are under the age of 16, you must attend your physiotherapy appointment with a parent or legal guardian.
